How You Can Contribute: The Health Adventure is a 501c-3 organization, and all donations are tax deductible. Clubs and individuals can make a one time donation, or pledge over three years. Clubs are encouraged to establish a goal and budget accordingly an amount appropriate to their size and membership. No amount is too large or small! After completing the contribution form, send it along with your check made out to The Health Adventure, and mail to:

Contributors to The Kiwanis Amphitheatre at the indicated giving levels will receive The David R. Burnette Service Medal in recognition of their gift. Those levels are: Bronze Medallion $500, Silver Medallion $1000, and Gold Medallion $1500. The David Burnette Service medal will be ordered, and sent for presentation to your club president. Allow 4 weeks to request and send your medal. Please indicate on the form the correct name to be engraved on the medal.

Carolinas District Kiwanis Foundation Giving: As an approved District Project, members can contribute funds toward The Kiwanis Amphitheatre project through the Carolinas District Kiwanis Foundation. At the $1000 level or above, members can be eligible to receive the Carl B. Hyatt Fellowship award, with checks made payable and sent to CDKF with the completed contribution form indicating the funds are designated for The Kiwanis Amphitheatre.
